Form MCS-150
The MCS-150, or Motor Carrier Identification Report, is a form required by the Federal Motor Carrier Safety Administration (FMCSA) for motor carriers to update their USDOT number company record.
Required Information
To complete this form, you need to include information such as:
- Your Legal Business Name.
- Your Doing Business Name (if it is different from the legal Business Name)
- Your Tax ID Number.
- Your Principal Place of Business Address.
- Your Mailing Address.
- The operations and cargo classifications of your business.
- Your carrier mileage for the last twelve months.
- The type of commercial vehicles in your fleet, the number of vehicles, and the number of drivers.
Biennial Update Schedule
Once you have your USDOT number, the last digits determine the year and month you need to file for the biennial update. The last digit of your USDOT number determines the deadline month:
- 1: January
- 2: February
- 3: March
- 4: April
- 5: May
- 6: June
- 7: July
- 8: August
- 9: September
- 0: October
When the next-to-last digit of its USDOT Number is odd, the motor carrier shall file its update in every odd-numbered calendar year. If the next-to-last digit is even, the update must be filed in every even-numbered calendar year.
As an example: USDOT #...45678 means file by August 31st. The next-to-last digit is 7 (odd), so file in odd-numbered years (2025, 2027, etc.).
